Our ID card is NOT an official proof of military service nor is it a guarantee of acceptance.

ELIGIBILITY

United States Military Veterans with character of service showing, Honorable, General, or Under Honorable Conditions. Absolutely no other character of service will be accepted for the ID card.

The applicant must have served in one of the five branches of military service, Army, Navy, Air Force, Marine Corps or Coast Guard. The only agency that can issue you a discharge is the branch with which you served.

Merchant Marines qualify with a valid DD-214 only.

WHAT YOU WILL NEED

1. An original or certified copy of a DD-214 for other than training

OR

An original or certified copy of an NGB-22 verifying completion of the first 6 or 8 year obligation in the National Guard or Reserves.

(If you do not have one, we can help you in requesting a certified DD-214 or NGB-22)

2. Verification with the Volume and Page number that the DD-214 or NGB-22 is recorded at the Wayne County Recorders

3. A valid Ohio Drivers’ License or State Photo ID card

If you do not have a DD-214 or NGB-22 you will not be issued an ID card.
